Good Food Delivered (GFD)

Step 1: Confirm Eligibility

The Good Food Delivered service is currently restricted to clients referred by clinicians/PHNs working in CHO6 Area 1 & Area 2.

Step 2: Read the Essential Information

Step 3: Complete a Referral

Please make a separate referral for each individual using the Making Connections Portal. If this is your first time using it, please read the guidance below first.

Read first – how to use the Making Connections Portal
  1. Creating a Referral Agent Account:
    To use the portal you will be prompted to first create a Referral Agent account.
    Q: What would you like to do? | Select: Make a Referral
    Q: Have you used the Making Connections Portal before? | Select: No
    Your details will be saved and you won’t have to re-enter them for future referrals.
  2. Using your Referral Agent Account:
    For all future referrals you will use your email address to access your account
    Q: What would you like to do? | Select: Make a Referral
    Q: Have you used the Making Connections Portal before? | Select: Yes
    Q: Please enter your email address to access your account | Enter your email address
  3. New Referrals:
    When you make a referral – you will be requested to enter the client’s surname & DOB. This step is required to to check for previous referrals or records that we might have for this person. You will see an cross or a tick beside the relevant programs. If the person has not yet been referred to us, you will be able to proceed.
  4. Program Selection:
    You can refer someone for one program at a time using the Portal
    Good Food Delivered
    or Befriending & Supports
  5. NB: Entering Dates
    Please note that the date format is MM/DD/YYYY
